PARENT STATEMENT AGREEMENT

 

You have our permission, in the event of an emergency and in case we are unavailable, to authorize any medical personnel to examine, interview, test and if necessary, treat my child as they may deem advisable.

I hereby give permission to the Tobago Hospitality and Tourism Institute (THTI) to photograph and/or videotape the student for educational or promotional purposes.

I hereby state that the child is in good mental and physical health condition to participate in the activities provided by THTI. I understand that THTI has the right to deny admittance to any student not meeting the standards of the program as it sees fit. I also agree not to hold these parties responsible in the event that my son/daughter/child engages in inappropriate conduct (including, but not limited to disruptive or volatile behavior in camp, etc.) or becomes involved in any activity or with any persons not associated with THTI, or its scheduled program and that THTI, has the right to send him/her home for inappropriate conduct without reimbursement of fees.

I further attest that the information contained in this application is correct to the best of my knowledge. In addition, I have agreed to the policy and fee statement and agree to comply.

NB. Children are to wear long pants and covered shoes. Slippers and or sandals would not be permitted.

Event Description:

The SEA Brain Cooler Camp, exclusively crafted for Standard 5 students embarking on a new chapter post-SEA Examinations. This dynamic program aims to nurture a love for culinary arts and baking among youngsters at an early age and provide an alternating way to educate and utilize their time as they await results for their next stage in life The camp will be held at our Mt. St. George Campus for a three-day period promising an enriching experience. Students would engage in hands-on baking and cooking sessions, refine dining etiquette, and unleash students creativity with mocktail making.

The camp would run on the following weeks and days from 9:00am to 1:00pm. Persons can select which week would be suitable for their child/ward and them.
