As part of the application process for entry into THTI, it is important that you provide accurate and up-to-date information regarding your health status.

This information will be used to ensure that appropriate support is provided to you during your time at the THTI, and to safeguard the health and wellbeing of all members of the Institute. Please be aware that providing false or misleading information on this form may result in the withdrawal of your offer of admission or other disciplinary action. The information you provide will be treated as confidential and will only be used for the purposes outlined above. However, in certain circumstances, such as where there is a risk to your own or others' health and safety, or where required by law, we may be required to share this information with relevant parties.
